TRANSCRIPT REQUEST FORM
A request for transcripts should be made at least 30 days prior to the deadline for mailing. Requests must be made in writing and accompanied by the correct processing fee. The fee for complete transcripts, inclusive of grades, recommendations, test scores, and awards or recognition is as follows:
MYP5 students – first 3 are free, any additional are $50.00 each
MYP1- 4 and former students - $50.00 each
Current students will be billed for courier fees
Former students must submit the courier fee at the time of request
Local courier - $40.00
North America courier - $60.00
UK or Europe - $65.00
Other - as determined
Please provide the following when submitting your request:
The correct information for the mailing address--courier services do not deliver to
P.O. Boxes and require a telephone number of the recipient.
Please make all checks payable to Somersfield Academy with “transcripts” noted in the memo section.
Include any forms or documents from the school requesting transcripts.
Please note that any omission will result in delay and this service is not always available during school holidays.
Note: ALL REQUESTS FOR TEACHER RECOMMENDATIONS MUST BE MADE THROUGH THE REGISTRAR
